The effectiveness of the application hot and cold fog generators using various different disinfectants

The paper presents results of a comparative study of bacteёricidal and sporicidal activity of hydrogen peroxide and Chloraminum B in the composition of hot and cold fog on the strain Staphylococcus aureus 906 and on spores of the strain Bacillus anthracis STI-1. The conditions were defined under which each fog generators embedded technical solutions have maximum disinfecting effect on rooms interiour surfaces. Authors present results of assessing the eficacy of application of modem disinfectants "Miracid", "Hort", "Hort light", "Hort steril concentrate" and "Perokcilakt" using a hot fog generator. The maximum antibacterial activity During disinfection of the interior surfaces of the room contaminated with bacteria of the strain S. aureus 906 and spores of the strain B. anthracis STI-1 by irrigation with disinfectants in concentrations of 8 % using a hot fog generator in concentrations, specified in the instructions for use, was detected in the disinfectant "Hort steril concentrate", which contains glutaraldehyde.

Effect of active substances of disinfectants at the stages of biofilms formation

With the systematic exposure of disinfectants (DS) to cholera vibrions, they can adapt by forming biofilms fixed on various surfaces. The aim of the work is to evaluate the effect of DS on the stage of formation of V. cholerae O1 and O139 serogroups biofilms on biotic and abiotic substrates. Materials and methods. Combined and monobiofilms were obtained according to previously developed methods, the concentration of V. cholerae of planktonic and biofilm forms was evaluated by bacteriological method and polymerase chain reaction in real time (PCR-RV). The statistical significance of the differences in experimental data was assessed using Student criteria. Results. At the stage of cell adhesion, cholera vibrions in the combined biofilm showed resistance to the action of ethyl alcohol (10 and 20 %), chloramine (0.1 and 0.2 %) at exposure up to 20 min. The bactericidal effect, expressed in the absence of culture growth and positive results in PCR-RV, was observed in samples with the addition of hydrogen peroxide of all dilutions, ethyl alcohol (50 %) and chloramine (0.5 and 1.0 %). At the monolayer stage, resistance to the action of ethyl alcohol of all dilutions, hydrogen peroxide (0.1 and 0.2 %) was revealed at exposure up to 20 minutes. Under the action of chloramine of all dilutions and hydrogen peroxide (0.5 and 1.0 %), the absence of V. cholerae growth on agar was noted with positive values of their concentrations in PCR-RV. The effect of all DS on mature biofilms maintained the previous pattern. Abio- and biotic biofilms of V. cholerae O139 ctxAB- and ctxAB+ showed resistance to ethyl alcohol of all dilutions, chloramine (0.1 and 0.2 %) at exposure up to 30 min. The bactericidal effect of hydrogen peroxide of all concentrations on abiotic biofilms of all strains and resistance of strains in biotic biofilms were noted. It is advisable to continue studying the effect of DS on vibrio cholera biofilms due to the possible "selection" of strains with increased resistance in the environment.

Analysis of disinfectants consumption in medicine facilities. Part 2. 2020–2022

The COVID-19 pandemic had an impact on all spheres of public health. The multiplied load on medical organizations could not but affect disinfection activities. In addition, given the closed borders in 2020, there was a shortage of both raw materials and ready disinfectants on the market. On the one hand, this situation has led to an explosive interest of the business community in the disinfectants market. This is evidenced by the increased number of disinfectants registered in the Russian Federation from 164 in 2019 to 467 and 471 in 2020 and 2021, respectively. In this study, we analyzed technical specifications for the procurement of disinfectants by medical organizations posted on www.zakupki.gov.ru. Despite the fact that during the period there were relaxations in the procurement legislation that allowed medical organizations to directly purchase disinfectants from suppliers, the analysis revealed significant deviations in 2020 and 2021 in the consumption of disinfectants compared to 2017–2019 and 2022. For example, the consumption of disinfectants based on sodium hypochlorite increased significantly. Also, with the shortage and increased cost of alcohol-based skin antiseptics, purchases of soaps with antibacterial properties have increased, which is especially noticeable in 2021. However, the 2022 purchases indicate that healthcare organizations are returning to normal operations and their consumption of disinfectants is becoming similar to that seen in 2017–2019.

Data on resistance of residential grey rats to 2nd generation anticoagulants

The chemical method of deratisation is the main means of regulating the numbers of synanthropic rodents. Low efficiency of deratisation measures can be related both to resistance of rats and mice to blood anticoagulants and to low attractiveness of baits. Thus, low eatability of baits entails insufficient cumulation of poison and, as a consequence, insufficient toxic effect on the animal organism. This paper describes the results of experiments to study the degree of sensitivity of grey rat individuals to the second generation anticoagulants bromadiolone and brodifacoum, most commonly used in deratisation practice. Sensitivity was determined by direct injection of anticoagulant solutions in neutral solvent into the stomach. The mean lethal concentration calculated by Hill’s equation for a single gastric administration was 0.35 mg/kg for bromadialone and 0.182 mg/kg for brodifacoum. The data obtained can be used in calculating the required amount of bait that must be eaten by the rodent to achieve lethality.

Features of compliance with the sanitary and hygienic regime by dog handlers of military units of the Russian Guard

The article presents the results of a comprehensive analysis of the organization of planned measures to comply with the sanitary and hygienic regime at the training complex of the cynological faculty of the Military Institute in the period from 2017 to 2023. The study shows that the cynological town is an artificial ecological system, which is a complex system of compartments (functional zones), the interaction of which, on the one hand, forms a continuous circulation of pathogenic biological origin, on the other hand, provides localization of processes, which, with proper organization of the sanitary regime, allows solving the problem of non-proliferation of infectious origin in the "man" system. – animal – epidemiologically significant surfaces, environments." As a result of the conducted research, the links of the epizootic chain in each functional zone of the cynological town are identified and described; the role and responsibilities of the daily outfit in maintaining internal order are formulated; the duties of military personnel to maintain the sanitary and hygienic regime at the cynological town are described; an algorithm for observing the sanitary and hygienic regime by the daily outfit and military personnel of the cynological unit in daily activities according to the main tasks is developed and areas of training of specialists of cynological services. The developed recommendations make it possible to adjust sanitary and hygienic measures at the dog training camp, are of great importance in solving health-saving issues and maintaining the livestock of dogs of service breeds.

Assessment of the economic efficiency of timely anti-epidemic measures in natural foci of HFRS

Within the framework of this study, on the example of the largest outbreak of HFRS in the Saratov region in 2019, the economic efficiency of anti-epidemic deratization measures in natural foci of HFRS was evaluated. The economic feasibility of timely implementation of anti-epidemic measures is substantiated. It is demonstrated that the delay in the implementation of an effective set of anti-epidemic measures, during the period of increasing morbidity, is associated with serious economic damage. It is shown that every day of "delay" in carrying out a set of measures during the outbreak of HFRS in the Saratov region in 2019 added 15 new cases of HFRS, which cost the budget 2,107,425 rubles daily. Calculations are given indicating that the implementation of timely anti-epidemic measures is highly cost-effective, since the benefits of the measures carried out significantly exceed the costs of their organization.

The risk of COVID-19 spread among medical workers of the Samara region in hospitals of various types

The purpose of this study was to assess the epidemiological risk for medical workers in the pandemic spread of a new coronavirus infection in various types of medical organizations. Materials and methods. The results of epidemiological surveillance of the staff of the State Budgetary Healthcare Institution of the Samara region "Samara City Clinical Hospital № 2 named after N.A. Semashko" in the number of 631 people, according to the protocols of 132 commissions for the prevention of infections related to the provision of medical care for the period from January 2021 to December 2022. Information from 1318 reports No. 1035 "Monitoring on the number of cases of coronavirus infection, including community-acquired pneumonia, and deaths" in the Samara region from the Rospotrebnadzor information system from April 22, 2020 to December 31, 2022 in terms of the total number of COVID-19 patients per day, the number of infected medical workers per day, the number of infected people in medical organizations. The results of the analysis of 11 scientific articles on the topic of the characteristics of epidemic foci in the teams of medical workers. Results and discussions. The COVID-19 pandemic has demonstrated the importance of developing national bed resources and the human resources of medical workers, taking into account the need for rapid conversion of somatic hospitals. Doctors, mid-level and junior medical personnel are at higher risk of infection both through contact with patients if they do not follow the rules for using PPE, and when interacting with each other. Repurposed somatic hospitals (GBUZ SB "SOKB named after V.D. Seredavina", FGBI VO "Clinics of SamSMU"), in comparison with a specialized infectious diseases hospital (GBUZ SB "GBS № 6"), due to the peculiarities of construction and planning solutions, represent an environment with a higher risk of infection of medical personnel, which requires additional organizational preventive measures. In addition to the general requirements of biological safety in the repurposed red zone, a comprehensive formation of a safe working environment is required, including such components as: organization of rest rooms for staff separately from the residents, development and observance of a meal schedule to eliminate the intersection of personnel flows as much as possible, digitalization of medical documentation, time-limited reception of biomaterial in the laboratory, issuance of the results of research through an electronic information system.
