The perspectives of scientific investigation in sphere of non-specific infection prevention, connected with medical treatment

In modern conditions of health development and humanity totally, the infection prevention, connected with medical treatment (ISMP) is the one of the global world problems. In Russia Federation there are no generalized proved statistical information about certain social and economic burden, which were made to people by ISMP. In Russia, concerning official statistics register about 25–30 ths. of ISMP cases annually, however native research workers consider that their real quantity is not less than 2–2,5 mln of people. The present statistics prove about misreporting of economic disbenefit from ISMP in Russia in 30–50 times. In modern conditions in Russia has accepted and prevail "National Concept of infection prevention, connected with medical treatment" (2011 year). The most important basis of practical realization of it state is scientific component. In the frame of present article are considered only some of the direction perspective scientific investigations in sphere of non-specific ISMP prevention. The problem of ISMP agents resistance to antimicrobials, including antibiotics, desinfectants (D) and skin antiseptics are situated to the present moment focused attention on scientific men and experts of practical healthcare in different countries. In the frame of present problem there is high-priority exploration of mechanisms of acquired resistance formation to biocides, concerning to results of genome sequencing, definition of D tests of ranking, according to the potential risk level of resistance formation to them, D rotation, learning new ways to creat D perspectives on the basis of synergetic combinations of antimicrobial agents and etc. One of the important modern scientific field is use of bacteriophage for ISMP problem solving. The most significance last years have got bacteriophage, because of the antibiotic resistance problem and nowdays they are high-potency used for treatment most part of purulent-septic infection nosological entity of different focalization, nosocomial enteric infection. This direction demands modern academic development for learning use of bacteriophage as biological disinfectants in epidemic focus of different ISMP and LPO nosological entities of different profiles.

Some ecological aspects of disinfectants use

Based on available data in Russia the most widely used active agents of disinfectants are cationic surface-active agents (surfactants), e.g. quaternary ammonium salts, triclosan, polyguanidines and phosphates. In recent decades, few attempts have been undertaken to evaluate the possibility of biodegradation of these compounds, as well as the effect of the active substances and products of their reduction on the environment. The development and wide use of multi-agent combined disinfectants, allowing expand the spectrum of their antimicrobial activity, require a detailed assessment of possible toxic effects of the disinfectants on the human health, on environment, as well as problems associated with possible formation of strains of chemical resistant microorganisms in health care. From an environmental point of view existing practice of widespread use of high concentrated disinfectants for high-level disinfection and sterilization of medical products is not justified. Clearly at the present moment there is a necessity to raise a question of search and the widespread introduction of the alternative chemical methods of high-level disinfection of endoscopes and sterilization of medical products, the development of the appropriate regulatory documents. Efficiency of some disinfectants against Bacillus anthracis spores

We’ve studied the sporocidal effect of 14 commercial disinfectants intended for decontamination of the external environment objects contaminated B. anthracis spores. It was determined that only 5 and 6 % concentration of chlorinated preparations had sporocidal activity concerning all studied test-surfaces, use of these disinfectants in less concentration (2 and 3 %) caused germination of spores on the dishes and death of laboratory animals.

Methods of analysis of chlorine dioxide (Analytical review)

Chlorine dioxide is widely used for water disinfection and bleaching of paper pulp. Recently, the Russian registered more based disinfectants chlorine dioxide, including medical facilities disinfection. Chlorine dioxide is analyzed using titrimetric, chromatographic and spectrophotometric methods. The most common method is titration with sodium thiosulfate in a neutral environment. To determine the chlorine dioxide in the workplace air ion exchange chromatography is used. Gas-chromatographic determination of chlorine dioxide with mass spectrometry is based on quantitatively reaction of iodine with 2,6-dimethylphenol, wherein the iodine is prepared by reacting potassium iodide with the chlorine dioxide. To determine the chlorine dioxide in water are widely used spectrophotometric methods using various indicators. Here noted that these methods are not sufficiently selective, as many attendant substances chlorine dioxide also react with these indicators and affect the assay results. For chlorine dioxide determination also used flow-injection analysis with luminescent detection, semiconductor sensors, and biochemical method using horseradish peroxidase. This paper presents a review of techniques and analysis of chlorine dioxide evaluated the possibility of their use in the disinfectants studying. New targets of medical disinsection

There is a short description of biology, inhabitations, injuriousness level: 17 species from 12 groups of arthropods, which have to be included to the list of “Targets of medical disinsection": Chironomidae, Gryllidae, Psocoptera, Forticula, Collembola, Thysanura, Psychodidae, Juliformia, Oniscoidea, Aranei, Pyralidae, Drosophillidae. There are ways of abundance’ limitation with help of providing sanitary and hygienic and internecine events and also explanatory work between people.

Mosquitoes and dirofilariasis

There was investigated partision of mosquitoes carrying-over dirofilariasis, which is promoted by Dirofilaria spp. – carnivore parasites and man. In dirofilariasis carry-over (larva helminth) takes part mosquitoes: Aedes (Stegomyia), Culex, Anopheles, Coquillettidia (Mansonia). The main problem of dirofilariasis is in cities with infested dogs, first of all because of the in-house and vagile dogs and mosquitoes mainly by genus Culex, which is carry-over reason of invasion by transmissible way year around. Conctrning to Rospotrebnadzor statistics, from 2006 were pointed out the tendency to expansion in the number of dirofilariasis cases between Russian people. There are represented the methods against mosquitoes by the help of imagocides, larvacides and protection of people and animals by the help of repellents.

Informational letter "Modern pediculicidal remedy against lices" (as of 01.01.2014)

Was represented records concerning pediculation disease incidences in RF. There are micro-population of lices, resisted to permethrin. At the same time were accidents of lices crossed resistance to permethrin and to different pyrethroids. Were represented negotiation’ ways of lices’ resistance to permethrin – comeback to organophosphorous compound (OC) and new formulation on the basis of benzyl benzoate, use of vegetable matter, organopolysiloxanes (dimethicones) and others. Was represented records about authorized for use remedies with insecticidal activity as related to lices, including to resistant to pyrethroids.
