Modern cleaning technologies in medical organizations positions, with epidemiological safety, hygiene and appropriateness economic efficiency

Infections associated with health care (HCAI) remain a global problem. One of the directions of their prevention – reduction of microbial contamination of environmental objects of the in medical organizations premises. Classic bucket method of cleaning and disinfection is not possible to obtain the desired effect. In this regard, in many medical institutions introducing modern cleaning techniques based on the use of new means of cleaning and the monitoring of its effectiveness. In three major General hospitals carried out expert assessment of the current cleaning technologies to epidemiological, hygienic and economic feasibility. The paper demonstrates hygienic and economic efficiency of modern technologies of cleaning of hospital environment. The study identified the factors influencing the epidemiological safety of the method and measures for their elimination. Use of a combination of Ag/AgCl nanocomposite and hydrogen peroxide for water disinfection in swimming pools

An experimental study was carried out to estimate the efficacy and toxicity of disinfectant containing hydrogen peroxide and Ag/AgCl nanocomposite. The possibility of disinfectant’s use for the treatment of all types of swimming pools was evaluated through in vitro and in vivo tests. It was found that in combination on the level of residual concentrations of hydrogen peroxide (0,8 mg/l) and silver (0,05 mg/l) is sufficient for complete water disinfection. The recommended dosages of application is not revealed harmful effects on the experimental animals.

Stability of bacteria having antibiotic resistance, to disinfectants

The article presents the results of a comparative study of antibiotic resistance and resistance to disinfectants of laboratory strains of different species of bacteria, long stored in the museum of microbial cultures with periodic reseeding on nutrient media containing selective antibiotics. We have detected a heterology of bacterial strains with plasmid and plasmid-chromosomal types of resistance to antibiotics in their sensitivity to disinfectants Mistral, Triloks, Brilliant, Detich, ammonium compaunds Treameen and Arquad 10–50, as well as to the matrix nanostructured antibacterial preparation Sky-Force. It is concluded that there is no correlation of formation of resistance to disinfectants of bacteria with polyantibiotic resistance, determined by genes of plasmid and chromosomal localization. This fact is essential to prevent the development of bacterial resistance to disinfectants based on experimentally proven their effectiveness, consistent with goals, objectives and rational tactics of disinfection measures.

New means of "NIID-AVIA" for extermination of the flying insects – carriers of causative agents of malaria and various fevers in insides of planes in the presence of people

Currently in our country is the important problem of treatment of civil aircraft of the against mosquitoes vectors of infectious agent of malaria, yellow, Chikungunya, Dengue, Zika fevers and others. Aircraft Civil Aviation performed a large number of international flights, including in the country, disadvantaged by a quarantine disease. The problem of pre-disinfection of aircraft arriving from countries dangerous for transmissible diseases; funds that are recommended for use on board and the relevant health standards. Developed by means of an aerosol "NIID-AVIA" on the basis of d-phenothrin, effectively destroying the flying insects and is safe for humans.

Investigation of Blattella germanica’ resistance to fipronil

Evaluation of phenylpyrazole sensitivity of cockroaches Blattella germanica (L.) revealed a high resistance to fipronil and pyriprole at 10-54 ×. For four of the six studied urban cockroaches subpopulations found a statistically significant slowing the onset of symptoms of poisoning by contact of insects with a glass surface, treated with fipronil (dose of 20 µg/cm2). Cockroaches subpopulations RM1, RM3 and ROBN were resistant to baits based on fipronil LT50 index (5,8 ×, 10,5 × 4,1 × and for males and 6.8 ×, and 8.6 × 9,5 × females, respectively). It was found a mosaic distribution resistant populations in the city. Sensitivity to fipronil RM4 population was similar to that laboratory strain SNIID, while the cockroaches collected at the public catering places, student’s homes or hospitals, showed high resistance to fipronil.

Insecticide resistance of Blattella germanica (L.) (Blattoptera: Blattellidae) (literature review 2000–2015)

There was represented evidence-based analysis of medical literature of Blattella germanica (L.) in the view of ability to transmit organisms that cause human diseases and allergen source. Around the world there are insecticide resistance formation between synanthropic insects. Concerning the results of committee on resistance of arthopods to insecticides (IRAC) Blattella germanica (L.) has resistance to 43 active ingredients of different chemical classes. There were considered questions of different resistance of arthopods to insecticides on larval and adult development stages, methods of resistance definition. There was analysed resistance of Blattella germanica (L.) to deltamethrin, beta-cyfluthrin, propoxur, chlorpyriphos, imidacloprid, fipronil, indoxacarb and others. There were represented the list of comparative sensitivity to insecticides of lab races in different countries, including Russia.

Efficacy of food baits for ant control

Ant control continues to stay the unsolved problem in urban and country environments. In urban environment, the Pharaoh ant is main pest, especially in healthcare institutions, food facilities, playschools and schools, whereas in countryside problems are associated with different species of garden ants. Experiments in natural conditions were conducted to evaluate the efficacy of food baits on the base of OPs (chlorpyrifos, diazinon, azamethiphos) and compounds from other chemical classes (fipronil, acetamiprid, deltamethrin, and sodium tetraborate) against ants. The best results were reached indoors against the Pharaoh ant by using gels (forced contact) and comparable ones – in natural experiments against some species of garden ants. Exactly evaluation of baits in natural conditions allowed to determining their attractiveness and efficacy.

Features of small mammal numbers and its epizootic significance in the natural infection foci in the Sarma river delta in autumn-winter 2015–2016

Investigations were performed in the Irkutsk region around Sarma river delta located in the Baikal lake hollow of Baikal-Dzhugdzhursk mountain-taiga area of Northern Asia in September 2015 and January-February 2016. Low number of small mammals was registered. Active long-tailed susliks (Urocitellus undulatus Pallas, 1779) were found in winter. Weather of summer and autumn 2015 was different from its characteristic long-term results: it was dry and lack of rainfalls. There was no snow cover at the background of considerable negative temperatures during registrations in winter 2015–2016. Possibly, these weather conditions negatively influenced on small mammal populations affected its number. In winter an Eastern-Asian mouse (Apodemus peninsulae Thomas, 1907) dominated in forest-shrubby habitats and a root vole (Alexandromys oeconomus Pallas, 1776) prevailed in near-water (damp, lengthways of waters) locations. Increase of small mammal number including the revealed dominating species is possible at environmental favourable conditions in summer and autumn 2016. It can be cause complication of epizootic situation in the natural zoonoses foci at the Baikal lake hollow. The last years there is a dynamical development of tourist branch around Baikal and increase in the number of holidaymakers. Rise of tourist visits (as a rule persons unimmunized to the agents of the natural focal infections) in nature can be cause aggravation of an epidemic situation. Therefore, it is necessary to constantly analyze the epizootic situation in the natural infection foci at the concerned territory to provide epidemiological surveillance and control, the timely organization of complex nonspecific anti-epidemic actions.
